THE ROLE

Coordinators are responsible for ensuring the delivery of high quality, person-centred services and supports by:

Being the key point of contact for the client

Being responsive to client needs and preferences through undertaking person-centred service planning processes

Responding to client complexity and escalating care needs

Establishing and developing relationships with the client, their network, and internal and external stakeholders

We require passionate, experienced, and versatile Care Coordinators able to:

Demonstrate an understanding of the Disability and Age Care requirements

Demonstrate experience in working as a service provider in the health and community services sector

Manage the delivery of high-quality services and outcomes for clients and ensure all services are delivered in a timely manner

Be self-motivated with the ability to work in a mobile, autonomous setting and as part of a successful team

You will need the following qualifications and experience:
Minimum of Diploma in Human Services discipline

Previous experience with the Human Services/Community Services sector

High level competency in the use of Microsoft Office (Outlook, Word, Excel)

The role is based in Adelaide and is full time at 76 hours/fortnight over 5 days/week.

The salary is based on SCHCADS Award Level 4.
